Although appointments are not required, please arrive prior to 3:00 pm to allow for document verification and processing time.

 

YOU WILL NEED:

1.  A certified copy of your Birth Certificate (not a mother’s copy or hospital copy) with the mother’s name listed.  No photocopies are accepted.  If you were born in Tennessee after 1975 you can get your TN birth certificate from our office . Click here for details for the TN Vital Records Office.  If you were born outside of TN, you can order a certified copy from www.vitalchek.com or www.vitalrec.com

2.  A valid Driver License or State Identification.  If your Driver License or Identification card was not issued in TN, please also bring your Social Security card

3.  Local checks or Money Orders

4.  Two (2) official Passport Photos which can be taken at our office for an $8.00 fee

5.  A Certified Marriage certificate if the last name differs from the birth certificate as a result of marriage

6.  Old passport book, if available

 All applicants must apply in person for a Passport.

GENERAL INFORMATION

The passport card facilitates entry and expedites document processing at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda. The card may not be used to travel by air. Otherwise, it carries the rights and privileges of the U.S. passport book and is adjudicated to the exact same standards.
A passport book allows to you to travel to many more locations via many more modes of transportation than the passport card alone. It is possible for you to apply for or have both the passport card and the passport book, although not necessary.

A Passport is valid for 10 years for a persons over 16 years of age and valid for 5 years for minors under 16 years of age.

Children under 18 years of age must have a parent or legal guardian with them when applying for a Passport.

EXPEDITED DELIVERY

If needing to expedite a passport (DS-11) application the following U.S. Department of State fees apply:

$110.00 for application fee,  $60.00 for expedited service, and $14.85  for mailing your new passport to you via overnight mail.  The total of $184.85 must be in the form of a check or money order and payable to the U.S. Department of State.  In addition, the following  Davidson County Clerk’s Office fees apply:

$25.00 execution fee, and $19.00 for overnight mailing to the Passport Agency.  The total of $44.00 may be paid to the Davidson County Clerk’s Office in any form.

If needing to submit a passport (DS-11) application without a rush the following fees apply:  A check for $110.00 payable to the U.S. Department of State for application fee plus $25.00 payable to the Davidson County Clerk’s Office for execution fee.

Routine (Renewal) Service – Form DS-82: Application for a U.S. Passport by Mail may be used if the conditions below are met.  Payment is non-refundable.

You may use Form DS-82 if your previous U.S. passport:

  1. Was issued when you were age 16 or older and
  2. Was issued in the last 15 years and
  3. Is undamaged and can be submitted with your application and
  4. Was issued in your current name or you can document your legal name change

Customers using Routine Renewal Service (Form Ds-82, not expedited) are responsible for mailing their applications directly to the U.S. Department of State.  Our office will NOT accept and mail Routine Renewal Applications.

Also, for renewals, a new passport photo will be needed.  Photos can be taken in the Davidson County Clerk’s Office for an $8.00 fee.

ACCEPTABLE FORMS OF PAYMENT

The U.S. Department of State will only take checks or money orders.  You should bring your checkbook or a money order for each applicant.  The Davidson County Clerk can accept any form of payment for its fees, including check, money order, cash, and debit/credit card.  A debit/credit card merchant fee of 2.3% is collected by a third party processor.

If you are using a hand carry/courier service, please note that a separate check or money order is still required for the US Department of State’s portion of the fees.

PROCESSING TIME

A passport will not be issued immediately.  The completed application will be sent from the County Clerk’s Office (or from a Post Office) to the Passport Services in the US Department of State in Charleston.

A “routine” passport application may take 4-6 weeks from the date of remittance.  This should be taken into consideration as travel plans are made.  For the most up-to-date processing times, please refer to http://www.travel.state.gov/passport/processing/processing_1740.html

The U.S. Department of State offers an expedited application option for an additional fee of $60.00 per application. Expedited applications have been processed and returned to the applicant in as little as 2-3 weeks.

If you need a passport sooner than two weeks, you may hire a passport courier.  An internet search can help you identify local passport couriers.  You will still need to apply in person and have your passport application assembled and sealed here or at any of the Federal Regional Passport Agencies.
